Note:  

The nominator is responsible for assembling the material required and submitting the original and required number of photocopies to the National Office c/o the awards manager. Only the limited items requested and listed below will be made available to the Selection Committee. Please ensure that the Nomination Form is complete. Facsimile transmissions of original material are not acceptable. Nominations shall remain in force for three years.
Send originals and six photocopies of:

1.  Nomination form (fully completed)
2.  Citation (maximum one page)
3.  Biographical sketch (maximum one page)
4.  Curriculum vitae and other pertinent information (maximum nine pages)
5.  Supporting letters (3 to 5) - Indicate if these are being sent directly
